- W1983010493 abstract "Recent advances in AD biomarkers have generated significant interest in biochemical, structural, and metabolic markers. Nonetheless, cognitive symptoms evaluated with psychometric tests form the core criteria of AD diagnosis and seem to outperform other markers in predicting progression to AD. Given the time-consuming nature of psychometric testing and the limited expertise in outpatient settings, ERP has emerged as a potentially useful biomarker of cognitive abilities. Application of advanced electronics and sophisticated analysis algorithms for ERP has made it possible to administer such tests in a physician's office in a standardized, user-friendly manner. Recent studies have demonstrated that wavelet decomposition of ERPs and subsequent training of ANNs can yield high classification accuracy (75-80%) between early AD and age-matched controls with sensitivity and specificity in the same range. The ongoing study aims to validate past ERP classification studies in a multi-center trial with a portable ERP system. In this ongoing study, 200 subjects (100 early AD, 100 controls) are being recruited across five sites. Each subject undergoes initial psychometric evaluation to exclude subjects with neurological disorders other than probable Alzheimer's disease (AD). Probable AD subjects undergo extended psychometric testing, blood testing, MRI volumetric analysis, and CSF analysis to support the diagnosis. Attached figure illustrates the testing schedule. A 2-deviant auditory oddball paradigm is being used for ERP testing. Wavelet decomposition of ERP waves will be used to train artificial neural networks (ANNs) to automatically classify AD from control subjects. Recruitment of 47 subjects (39 controls, 8 ADs) has been completed. As shown in attached table, preliminary analysis of 8 AD and 8 age- and education-matched control subjects reveals a statistically significant difference in the latency of P300 peak at electrode Fz in response to the distracter tone (p < 0.05). It is expected that sufficient number of AD subjects will be recruited by June 2012 to enable training of ANNs based on ERP data. Preliminary analysis of ERP data demonstrate a promising trend in P300 latency for discriminating between early AD and healthy controls. Depending on recruitment, classification data will be presented along with updated statistical analyses." @default.
