• W1983180132 abstract "Background: Magnifying endoscopy has a potential to evaluate tissue character based upon tissue structural atypism. Recent development of NBI-enhanced technology enables to observe not only pit pattern changes but also vascular structural changes. The present authors advocate “APC score” as differential diagnostic criteria for gastric neoplastic lesions based upon NBI-enhanced magnifying endoscopic findings. Methods: 234 consecutive cases which were classified into A1 or A2 (196 cases of gastric neoplasm and 38 cases of gastritis) were included in this study. These lesions were observed with NBI-enhanced magnifying endoscopy (GIF-240Z, -260HZ, Processor Olympus EVIS Lucera, Spectrum) and were scored by newly designed “APC score”. “APC score”: APC score is composed of three categories of area formation “A”, gastric pit pattern “P” and capillary pattern “C”. Each category is subdivided into three types, respectively. Area formation is classified into A0 (no area formation), A1 (area formation with unclear border) and A2 (area formation with clear border). Pit pattern is classified into P0 (regular pit), P1 (irregular pit) and P2 (destructive pit). P2 is subclassified into P2a (irregular and crowded), P2b (partially non-structure) and P2c (non structure). Capillary pattern is classified into C0 (regularly running), C1 (slightly irregularly running) and C2 (highly irregularly running). C2 is subclassified into C2a (network pattern), C2c (corkscrew pattern) and C2b (unclassified). Results: Gastritis is scored in A1; 94.7%, P0; 57.9%, C0; 42.1% and C1; 42.1%. Adenoma is A2; 76.9%, P1; 84.6%, C1; 53.8%. Differentiated adenocarcinoma is A2; 81.8%, P2a; 60.0%, C2a; 64.2%. Undifferentiated adenocarcinoma is A2; 59.1%, P2c 59.1%, C2c; 77.3%. Conclusion: APC score is considered to be useful to describe tissue character of gastric lesions appropriately by NBI enhanced magnifying endoscope." @default.
