- W1983533615 abstract "Embryo transfer (ET) is the final and most crucial step in IVF. While the pregnancy rate (PR) after ET is dependent upon multiple factors that include patient age, an additional factor includes the physician performing the transfer. It is well documented that different physicians have different PRs after ET, especially when examining the most successful patient groups. The present analysis was undertaken to determine whether such variations among physician transfer rates persist for patients over the age of forty years, a subgroup known to have lower PRs from IVF. Retrospective analysis of IVF pregnancy rates for individual physicians performing the ET at Boston IVF. Pregnancy rates were calculated per physician as the number of pregnancies per number of patients undergoing an embryo transfer and were analyzed for two groups of patients: women <35 years of age (Group A) and 40 years and over (Group B). All patients had ≤3 fresh non-donor IVF cycles between January 1999 and December 2003. Pregnancy was defined as an hCG level > 100 mIU/ml. Pregnancy results among the physicians were compared by X2 analysis. P<0.05 was considered statistically significant. A total of 5280 IVF cycles were analyzed. Among the 12 physicians, the PRs in Group A ranged from 26.9% to 37.9% (P=.381). PRs between the top ranked doctor and the bottom ranked doctor for these patients were statistically different (P=.007). For Group B, the PR ranged from 11.1% to 17.0% (P=.775); PRs between the top and the bottom ranked doctors were not significantly different (P=.078). There was no consistency between rank order of the physicians when comparing Groups A and B: physicians ranking 1,2,3, and 4 for the younger group ranked 10, 9, 7, and 8 for the older age group and physicians ranking 12, 11, 10, and 9 for Group A ranked 5, 2, 3, and 4 for group B. Prior studies have demonstrated that factors known to affect IVF pregnancy rates include patient age, embryo quality and embryo transfer technique. While our data confirm the importance of embryo transfer technique for women <35 years of age, surprisingly, this impact was not evident in women aged 40 and over. Our data suggests that as pregnancy rates precipitously drop after age 40, the patient's inherent fecundity obscures the impact of physician transfer technique." @default.
