- W1983755076 abstract "Sir—E Quinlivan and colleagues (Jan 19, p 227) report that folic-acid supplementation, at doses of up to 500 g daily, diminishes the dependency of plasma homocysteine on plasma folate, but strengthens the inverse association between plasma homocysteine and plasma vitamin B12. However, on scrutiny of the data we suggest the analysis may need to be revised. Our data, from a comparable study, show conflicting results. Quinlivan and colleagues show poor agreement between correlation and slope values for the regression of log homocysteine and log vitamin B12. There is also substantial disagreement between quoted correlations and such as lysophosphatidic acid and osteopontin. The existence of multiple sets of discriminatory patterns in the serum proteome is in keeping with our hypothesis that the discriminatory proteins or peptides emanate from the specific tumour-host microenvironment, especially in the earliest stages of cancer. Although E Diamandis states that it is unlikely that such epiphenomena will be specific for one type of cancer, we propose and show the opposite. As we stated in our report, ovarian and prostate cancer exhibit unique non-overlapping patterns. Perhaps the best biomarkers for early detection will not even be tumour-derived molecules. Cancer is now understood to be a product of the tumour-host microenvironment. This unique and organ-specific milieu can generate, and enzymatically amplify, multiple proteins, peptides, metabolites, and cleavage products at much higher concentrations than for molecules derived only from the tumour cells. Diamandis claims that proteomic patterns should be based on proteins of known identity. Our data show, to the contrary, that the diagnostic usefulness of proteomic pattern technology is independent of the underlying identity of the proteins.
