These immunologic techniques have also proved vahtable as investigative tools in the study of cutaneous lupus erytltematosus (LE) and the cutaneous manifestations of vasculitis. The classic discoid LE lesion is sharply demarcated, erythematous, and generally round, showing adherent scale formation, follicular plugging, telangiectasia, hyperpigmentation and hypopigmentation, and scar formation (fig. I). These lesions involve predominantly tire sun-exposed areas of skin and may be accompanied by features of systemic lupus erythematosus (SLE). Biopsy specimens ofctt taneous LE lesions stained with hematoxylin-cosin show orthohyperkeratosis, patchy epidermal atrophy, and vacuolar alteration of the basal cell layer (liquefaction degeneration) (fig. 2). A patchy mononucicar cell infiltrate bugs the dermal-epidermal junction and surrounds the dermal appendages, i.e., the sweat glands, sebaceous glands, and hair follicles. In addition, prominent perivascular mononuclear cell infiltrates are common in both the papillary and the reticular areas of the dermis. These histologic featnres are also seen in the classic butterfly rash of LE. In general, a cutaneous lesion arising from cutaneons LE cannot be distinguished from a cutaneous lesion arising from SLE on the basis of clinical and histologic findings. Recent evidence, however, indicates that in many patients wittt LE wlto have cltaracteristic pronounced photosensitive, widespread annular, polycyclic LE dermati t is and possess the Ro(SSA) antibody, there is also distinct, intense vacuolar alteration of the basal cell layer 4 (figs. 3 and 4).
