- W1984569621 abstract "Maternal cigarette smoking is a leading cause of fetal growth restriction (FGR). This global restriction of growth is thought to be due to intrauterine carbon monoxide and nicotine exposure and direct effects of toxins at a cellular level. Smoking is protective against the development of pre-eclampsia (PET). As abnormal UA and MCA Doppler waveforms are independent predictors of morbidity in FGR we aimed to examine the effect of cigarette smoking on Doppler patterns and perinatal outcome in a large cohort of fetuses with EFW <10th centile. PORTO recruited 1,116 non-anomalous singleton pregnancies with EFW <10th centile. All fetuses were subjected to intensive serial sonographic surveillance until birth. Adverse perinatal outcome was defined as a composite outcome of IVH, PVL, HIE, NEC, BPD, sepsis, and death. This secondary analysis compares smoking and non-smoking mothers with respect to pregnancy characteristics and perinatal outcome. A significant proportion of FGR fetuses (23%) enrolled in the PORTO study were subjected to intrauterine nicotine exposure. Smoking mothers were significantly younger (p=0.009), lighter (p=0.001) and consumed any alcohol (p<0.001) during pregnancy than non-smoking mothers. They had significantly lower rates of PET (p=0.007); women who smoked more commonly had babies with birthweights <10th centile when compared to those who did not smoke. There was no difference in the presence of oligohydramnios or abnormal UA Doppler however there was a significant difference in MCA abnormalities between the groups. Maternal cigarette smoking resulted in a significantly lower incidence of MCA Doppler abnormalities (p=0.005). Our data confirm known effects of maternal smoking on fetal development and pregnancy outcome. Interestingly, antenatal nicotine exposure resulted in less MCA Doppler abnormalities, possibly due to its vasodilatory effect which further supports a different etiological pathway of growth restriction in this subgroup of mothers." @default.
