Although these diseases commonly present as comorbid conditions, numerous epidemiological and observational studies have shown depression to be an independent risk factor for the development and progression of IHD. Patients with depression are more likely than those without it to develop IHD, and patients with depression who have IHD are more likely than those without depression to die of or have a recurrence of cardiac events. Thus, a challenging task for the medical profession is to identify ways to prevent or at least modify the adverse association between depression and IHD. Understanding this association is increasingly important to clinical researchers, and the knowledge gained in this area will have important practical clinical applications. In the past, it seemed reasonable to assume that a treatment targeting improvement in the symptoms of depression might also result in a better prognosis for depressed patients with IHD. However, several recent randomized clinical trials with large sample sizes have tested this assumption, and their findings are far from promising. These studies challenge investigators and trial design experts to create new approaches for future trials. Data gathered from new trials should be pertinent to the larger patient population, allowing physicians to apply these data in clinical practice. This series in the Journal presents the proceedings of a symposium held in Baltimore, Md, in January 2002.
