Since most studies have focused on estimating prevalence of UI at a single time point, our understanding of its development and natural history is limited. The goal of this study is to describe the long-term time course of UI in women with type 1 diabetes (T1DM). METHODS Longitudinal data from 604 females in the Epidemiology of Diabetes Interventions and Complications (EDIC) study, an epidemiologic observation follow up of the Diabetes Control and Complications Trial (DCCT) cohort, defined UI annually over 7 years (EDIC years 13 through 19) as a “Yes” response to a single question querying presence of urinary incontinence. No UI was defined as “No” to the same annual question. Subjects were categorized into 4 groups: No UI, isolated UI for a single event of <=2 years duration, episodic UI for subjects flipping between UI and no UI at least twice over study period, and persistent UI for subjects never returning to a state of no symptoms. RESULTS Average age at EDIC year 13 in this study group was 46 years. Prevalence of UI increased from 18.1% (n=102) at EDIC year 13 to 32.4% (n=187) by EDIC year 19; 49% of women never reported UI during the 7 year follow up. Fifteen percent reported persistent UI, 14% reported isolated UI and 22% reported episodic UI. Likelihood of remission decreased with every additional year of prior reported UI, from 41% chance after first report of UI to only 15% for those with 4 consecutive years of UI (Figure 1). CONCLUSIONS UI is a dynamic state in women with T1DM that may be modifiable. Studies describing factors related to its clinical course are underway.
