- W1987201468 abstract "Intestinal metaplasia (IM) arising near the gastro-oesophageal junction (GOJ) is one of the most controversial topics in gastrointestinal pathology. Much of the confusion lies in the difficulty in precisely identifying the GOJ by endoscopy, since it is critical to know the relationship of the site of the biopsy specimen to the squamocolumnar junction and the GOJ. It is often difficult for the pathologist to determine if IM present in the biopsy specimen represents ultrashort-segment Barrett's oesophagus (columnar-lined oesophagus) or IM of the gastric cardia/proximal stomach. IM in these sites typically arises in an inflammatory background (so-called ‘carditis’), although there are at least two forms of ‘carditis’ adding further to this confusion. Although data are sparse, there is a suggestion that the risk of progression to dysplasia/adenocarcinoma is significantly greater if the IM is arising in the tubular oesophagus compared with the gastric cardia/proximal stomach. Should future data support this contention, it will become incumbent upon the pathologist to determine the origin of IM in biopsy specimens from this site." @default.
