Crystalloid solutions were developed to replace volume losses and augment the circulatory system by reproducing the composition and tonicity of human intravascular extracellular fluid. Following the basic principles that sodium drives the tonicity, and water volume will follow the sodium, the logical next step in improving the volume expanding capability of a crystalloid solution was to increase the sodium concentration. What was not obvious or apparent were the profound and complex effects that such a seemingly simple and benign alteration could have. The varied and often unexpected systemic effects of standard crystalloid solutions, such as lactated Ringer's or normal saline, have only recently become fully appreciated. Arguably, the most important of these findings has been that crystalloid solutions act as major immunomodulators and can significantly augment multiple deleterious or injurious pro-inflammatory pathways." @default.
