- W1989227623 abstract "A pilot investigation to evaluate differential threshold (low, medium, automatic) of pre-programmed actigraphy (ACG) epoch intervals (15 seconds, 30 seconds, 60 seconds) to sleep during overnight polysomnography (PSG) in minimally responsive patients. Case series. VA Polytrauma Rehabilitation Center. Participants included three males evaluated during inpatient brain injury rehabilitation. Participants were Caucasian with a mean age of 30 (range 25-37), and severe TBI injury (mean initial GCS=4; range 3-5). All participants had significant spasticity, muscle weakness, and were minimally responsive or acutely confused at the time of PSG and ACG dual recordings. Not applicable. PSG/ACG total sleep time (TST), PSG/ACG sleep efficiency (SE) & PSG/ACG wake after sleep onset (WASO) using Respironics-2 Actiwatch and Somnostar 9.1 Polysomonogram with Viasys Program Software. Both PSG and ACG were well tolerated. For Case 1, ACG failed to detect sleep at 15 and 30 second epoch intervals regardless of threshold used. Using the low and automatic thresholds to analyze 60 second epochs, ACG detected sleep, but overestimated sleep by 95 minutes when compared to PSG (Case 1 PSG-TST = 253.5 minutes, PSG-SE = 70%, PSG-WASO = 103.5 minutes). For Case 2, ACG failed to detect sleep across all intervals and thresholds (Case 2 PSG-TST = 51.5 minutes, PSG-SE = 13.1%, PSG-WASO = 18 minutes). For Case 3, ACG overestimated sleep across all three epoch intervals and thresholds by approximately 76 minutes (Case 3 PSG-TST = 302 minutes, PSG-SE = 76.8%, PSG-WASO = 74 minutes) with little variation across thresholds or epochs for TST or SE but not WASO. These cases illustrate the need for further research into ACG and PSG agreement across epoch and threshold settings within severe TBI populations with spasticity and impaired manual muscle strength whose impairments influence movement detection used for ACG readings." @default.
