Recently, new strategies to improve cancer control using existing therapeutic agents have been suggested (1). The overall goal is to modify the physiological state of tumour cells so that a therapeutic gain can be achieved with conventional treatment such as radiotherapy and chemotherapy (2-5). Poor blood flow through regions of a tumour may reduce access to chemotherapeutic agents and targetted antibodies (6) and high interstitial tissue pressure further restricts the concentration of anticancer agents within tumours (7). A consequence of reduced perfusion is poor oxygen delivery which results in hypoxia. This severely compromises the effectiveness of conventional radiotherapy and may lead to loss of local control, drug resistance and metastatic disease (8-10). Tumour cells may become either chronically hypoxic (secondary to low vascular density, anaemia or low perfusion pressure) or acute/y hypoxic (secondary to transient changes in perfusion of the tumour vascular bed) (9). Pharmacological agents can be used to eliminate acute hypoxia by maintaining perfusion through the tumour vascular bed. This optimizes radiotherapy efficacy and allows penetration of chemical modifiers toxic to the chronically hypoxic cells. Re-oxygenation may be a double-edged sword because chronically hypoxic cells" @default.
