- W1990094323 abstract "The benefits of eSET have been elucidated in multiple studies yet the practice remains difficult to implement in the U.S. despite this research and ASRM guidelines. We report clinical pregnancy (CPR), live birth (LBR), and ongoing pregnancy (OPR) rates in pts undergoing fresh eSET in addition to subsequent frozen ETs (FETs) to deduce a cumulative CPR (CCPR) from this initial cycle. Retrospective analysis of autologous IVF cycles conducted from 2009-11 at a U.S. university-based center. We identified pts who underwent eSET, defined as transfer of a stage 2 or better blastocyst when additional embryos were also available for transfer. CPR (fetal cardiac motion), LBR, and OPR were collected for fresh ETs and subsequent FETs. CCPR was calculated; the rate of multiple gestations was noted. Double ET (DET) cycles from this time period were used for comparison. 919 cycles had eSET/DET where a stage 2 blast or better was transferred; 139 cycles had eSET (15%, mean age 33.8y) while 780 had DET. 114 of eSET cycles had embryos cryopreserved on day 5/6. eSET achieved equal CPRs compared with DET, but nearly eliminated the risk for twins (2 monozygotics occurred in the eSET group). Of pts with cryopreservation, the CPR was 65% from the initial transfer. Furthermore, a CCPR of 69-85% was attained. However, it did significantly decrease the risk for multiple gestation, which is known to present numerous risks during pregnancy. Good prognosis pts (particularly those with cryopreservation) should be encouraged to consider eSET, as is standard, and even mandated, in many countries outside the U.S." @default.
