- W1990646145 abstract "To evaluate whether there is any difference in embryonic development and clinical pregnancy outcome in single blastocyst embryo transfers, when rec-HCG is administered for final oocyte maturation instead of the standard regime of urinary HCG. Prospective randomized controlled trial. Intension to treat analysis was performed. Inclusion criteria were: (i) age less than 36, (ii) FSH <12 IU/ml, (iii) ejaculated sperm origin, (iv) signed informed consent (v) elective single embryo transfer on day-5. Exclusion criteria were: (i) known intra-uterine pathology (ii) testicular sperm (iii) endometriosis stage III & IV. 119 patients were randomised either to receive 250 mcg rec-HCG or 10.000 IU u-HCG for ovulation triggering. Ovarian stimulation was performed with a fixed dose 187.5 IU for 5 days of recFSH starting on day 2 of the cycle, with co-administration of 0.25 mg cetrorelix commenced on day 7. Ovulation triggering medication was administered as soon as 3 follicles of 17 mm were present. Overall, 59 patients in rec-HCG group and 60 in the u-HCG group were randomised and analysed. Demographics, stimulation and embryological parameters were comparable between the rec-HCG group and the u-HCG group: age (29.5 vs. 29.7 years, P=0.8); duration of stimulation (9.6 vs. 9.6 days, P=0.9); COCs retrieved (13.2 vs. 12.0, P=0.3); 2PN embryos (8.3 vs. 6.9, P=0.08); mean embryo quality (1.3 vs. 1.3, P=0.8); blastulation rate (60% vs. 59%, P=0.9); cryopreserved embryos (2.8 vs. 2.6, P=0.8) respectively. Positive HCG test was similar in both groups 45.8% (n = 27) vs. 41.7% (n = 25). The ongoing pregnancy rate (>12 weeks) was statistically significantly higher in the rec-HCG group 45.8% (n = 27) compared to 26.7% (n = 16) in the u-HCG group, P=0.03, [Odds Ratio: 0.43, 95% CI: 0.20–0.92]. Higher pregnancy loss rate observed in the u-HCG group 28.0% compared to 0% in the rec-HCG group (P=0.001) The current RCT shows that patients may achieve higher ongoing pregnancy rates when ovulation is triggered by 250 mcg recombinant HCG instead of the standard regimen of 10.000 IU urinary HCG. In the absence of any superiority of the single blastocyst transferred with regards blastulation rate and mean embryo quality, an endometrial factor and/or any endometrial/embryo paracrine interaction might be assumed. Taking into account that the dose of rec-HCG is equivalent to 6700 IU of the urinary product the dogma of 10000 units in ART should be reviewed" @default.
