- W1990960467 abstract "To compare the perineal structures identified by ultrasound during late pregnancy with those ultrasonically visualized immediately after an anal sphincter tear at delivery. Forty-five nulliparous women in late pregnancy (32–40 weeks); 40 women with clinically diagnosed sphincter tears. Vaginal and anal scans were performed using a biplane rectal probe (Hitachi U553) while the semicircular probe was retracted. In addition, a transvaginal sagittal and a series of oblique images of the perineal structures and the anal canal were done with a linear array transducer. Two observers evaluated the possible presence of perineal structures in the midline sagittal transvaginal scan. The ability to identify the perineal structures in pregnancy and after a sphincter tear is given in the Table. There was very good agreement between the two observers during pregnancy; the agreement was less good regarding the remaining structures after injury. The median area of the external anal sphincter (EAS) during pregnancy was 0.89 cm2 (range 0.52–1.46), the length and height 14.9 (5.7–20.3) and 7.9 (4.9–10.8) mm, respectively. During pregnancy, the midsagittal vaginal linear scan identified the EAS, internal anal sphincter, puboperineal muscle and the perineal membrane. The central point and longitudinal anal muscle were not reliably identifiable. The midsagittal image was fair for identifying a partly torn EAS, but had a low interobserver and inter-methods reliability for the other structures.
