You point out that this incident is another blow to confidence in Japan's medical system. We agree, and believe that this case reflects certain issues with the health system itself. We have two types of nurses in Japan—registered nurses who have passed a national board examination and who are licensed by the federal government, and assistant nurses who are licensed by the local prefecture. Registered nurses study for 3 years at nursing school, whereas assistant nurses study for only 2 years. Although the practical training and educational achievements differ between the two types of nurses, an assistant nurse is permitted to do the same type of work as a registered nurse and hospitals expect the same level of performance from both types of nurses. However, assistant nurses are paid about 20% less than registered nurses. In a 1996 report, the number of assistant nurses was similar to that of registered nurses. However, especially in hospitals running a deficit, such as Hokuryo Hospital, it has become increasingly common to lower costs by employing more assistant nurses. Only a small percentage of nurses in Japan are men, most of whom work in mental hospitals. Male nurses in other hospitals find promotions difficult to obtain because the chief nurses are generally women. Accordingly, male assistant nurses have the lowest status among nurses. We wonder therefore at the atisfaction of these male nurses with the job situation. Such perceived and real inequities in pay and responsibilities, as well as disparities in education and training, have undoubtedly contributed to difficulties in the Japanese medical system, and might continue to be ggravated by the continuing economic stagnation.
