- W1990992753 abstract "Even though ovarian cryopreservation has been performed over a decade, no data exist on the long term follow up and the utilization of the cryopreserved tissue. Our purpose was to analyze the indications and safety of ovarian cryopreservation and provide a perspective on the long-term utilization of frozen-banked ovarian tissue. Prospective. 55 females underwent ovarian tissue freezing with a slow freezing technique between 5/97 and 3/07. Follow up was by phone with a response rate of 91%. Indications are summarized in the table. Age range at freezing was 4–44. Eight patients also underwent oocyte or embryo freezing 5–63 days prior to ovarian tissue freezing. 50 cases were performed laparoscopically and in 5 tissue was harvested during surgery for cancer treatment (n = 4) or cesarean delivery (n = 1). In two, frozen ovarian tissue was transferred to our unit. In all cases at least an ovary was removed. In one, biopsies were also obtained from the contralateral ovary, and bilateral harvest was done in another patient. There were no surgical complications and no ovarian metastasis was found on histology. Three patients had thrombocytopenia at the time of laparoscopy (36,000–48,000/mL) with no bleeding during surgery. Median length of storage was 4.02 ± 2.8 years (range of 0.17–10). Of those, only 3 (5.5%) underwent transplantation, two with heterotopic (abdominal wall) and one with the orthotopic technique. Of the two heterotopic transplant patients, one had an embryo transfer which did not result in a pregnancy and the other spontaneously conceived twice. While the first pregnancy was not viable, the second resulted in a healthy baby who is currently 2 years of age. 94.5% of patients have not yet utilized their tissue (table) but 2 are contemplating transplantation. The indications for ovarian tissue freezing are diverse but the tissue harvesting procedure appears to be safe. 73.5% of those who underwent ovarian cryopreservation developed amenorrhea indicating the appropriateness of fertility preservation.
