- W1991051331 abstract "Oral rehydration, begun during the early stages of diarrhea, is effective in preventing dehydration, which often causes death. Experience from many parts of the world has shown that the solution to be administered can be prepared at negligible cost by the mother using materials available in the home. Moreover, mothers both literate or illiterate can be taught to prepare and use the solution effectively. There is growing concern at the appearance of numerous varieties of commercial oral rehydration solutions and sachets of salt mixtures in the market. Reasons for concern are as follows. 1) Mothers will become dependent on the preparation for treatment of the diarrhea. 2) Self-reliance, a tenet of primary care, will be undermined as mothers will no longer see the need to acquire solution preparation skills. 3) Shortages of the commercial preparation, like the shortage of other drugs in many clinics, will prevent the early treatment of diarrhea. 4) The cost of treating children with diarrhea will be many times greater to the mother if she uses proprietary rehydration products. 5) Commercial preparations have a shelf life of only 3 months which can be extended to 1 year in cases where special packaging is used; however the cost of the sachets will then increase. With oral rehydration, healthworkers in many countries have the unique opportunity to transfer a simple medical skill to the members of a community, thus making the community self-reliant in the management of early gastroenteritis. The advent of commercial preparations will return us to dependence on drug firms. There is an analogy here with breastfeeding, where mothers with ample breastmilk have been persuaded through advertising to feed their children with formula." @default.
