We hypothesized that allergen-induced basophil activation would be suppressed in patients receiving peanut SLIT. METHODS: Whole blood specimens were collected at baseline and at 12 months from peanut allergic patients participating in a double-blind, placebo-controlled, randomized trial of peanut SLIT. Unfractionated cells were stimulated with a 4-log range of peanut extract (1μg/mL- 1ng/mL) and controls (anti-IgE and basophil media). Basophils were identified by flow cytometry (CD123+, CD203c+, lineage -) and activation status defined by CD63 expression. RESULTS: Eleven patients were assessed for basophil activation at baseline and after 12 months of treatment. At baseline, there was no statistical difference in basophil activation upon peanut-induced stimulation at 0.01μg/mL between the two groups (placebo n=5, median 12.6% (0.4-38.3%) CD63+ vs treatment n=6, median 6.8% (1.6-52.5%) CD63+, p=0.79). At 12 months there was a statistically significant decrease in allergen-induced basophil activation at 0.01μg/mL in the active treatment group (median 1.25%, (0.3-9.4%)) compared to the placebo group (median 62% (5.2-72.9%), p=0.009). A similar difference was observed after stimulation with 1ng/mL of peanut (p=0.75 at baseline, p=0.009 at 12 months). Stimulation with anti-IgE as a control produced no difference (p=0.25). In this small sample size, basophil suppression did not seem to correlate with the amount of peanut protein tolerated during oral food challenge. CONCLUSIONS: Peanut SLIT suppresses allergen-induced basophil activation that may contribute to clinical desensitization. Whether peanut SLIT induces immunologic tolerance and what role basophil suppression might play requires further study." @default.
