Gli autori del lavoro, recentemente estensori e collaboratori – su mandato della Società Italiana di Chirurgia Maxillo-Facciale (SICMF) e della Società Italiana di Patologia e Medicina Orale (SIPMO), delle Raccomandazioni clinicoterapeutiche sull’osteonecrosi delle ossa mascellari associata a bisfosfonati e sua prevenzione – hanno esaminato le informazioni scientifiche disponibili in merito alla ONJ (associata all’assunzione sia di bisfosfonati sia di denosumab e/o anti-angiogenetici) in rapporto con le procedure odontoiatriche. La ONJ da bisfosfonati, ma anche, negli ultimi anni, da denosumab e anti-angiogenetici, si può considerare la più emergente delle patologie odontoiatriche severe: negli ultimi 10 anni si sono registrati migliaia di casi in letteratura, con importanti implicazioni sulla qualità di vita del paziente. La comunità medica e odontoiatrica ha percepito la severità della patologia, ma anche la necessità di darsi regole, di ridisegnare il percorso preventivo e assistenziale di tale malattia: in tutte le nazioni industrializzate e parimenti in Italia sono state stilate linee guida o raccomandazioni per la prevenzione e cura della ONJ, malattia inizialmente solo associata ai bisfosfonati per via endovenosa, in pazienti oncologici, ma poi rilevata, sebbene con frequenza inferiore, anche nei pazienti non oncologici, principalmente osteoporotici, in terapia con bisfosfonati. Al fine di ridurre il rischio di ONJ, il ruolo dell’odontoiatra è fondamentale: dall’eliminazione dei fattori di rischio locali noti all’applicazione dei protocolli di gestione odontoiatrica, alla scelta del timing dell’azione odontoiatrica, sia per il paziente oncologico (prima, durante e dopo il trattamento con bisfosfonati in combinazione o non in combinazione con anti-angiogenetici o con denosumab) sia per il paziente non oncologico (prima, durante e dopo il trattamento con bisfosfonati). Specific protocols are suggested for the dental management of patients taking bisphosphonates, denosumab or antiangiogenic agents so as to reduce the risk of osteonecrosis of the jaw (ONJ). The authors, directly involved with the recent publication of the Raccomandazioni clinico-terapeutiche sull’osteonecrosi delle ossa mascellari associata a bisfosfonati e sua prevenzione, endorsed by Società Italiana di Chirurgia Maxillo-Facciale (SICMF) and Società Italiana di Patologia e Medicina Orale (SIPMO), survey and discuss the dental management of patients taking bone modifiers and/or anti-angiogenic agents at risk of ONJ, on the basis of available scientific information. ONJ should be considered the most emerging disease that affects the oral cavity; ONJ is mainly due to bisphosphonates, but it has been also associated with denosumab and anti-angiogenic agents in recent years. Thousands of cases have been reported in the scientific literature so far, with important implications for the National Health Systems. ONJ can heavily impact on patients’ quality of life. In all industrialized nations and in Italy too, guidelines or recommendations for the prevention and treatment of ONJ have been drafted. In the beginning, these documents were centred on the care of cancer patients taking intravenous bisphosphonates, but soon after they were upgraded to include the less-frequent ONJ in non-cancer patients taking bisphosphonates. To reduce the occurrence of ONJ in patients at risk, the role of the dentist is crucial in the effort to eliminate the known local risk factors applying specific dental treatment protocols, wherever indicated, and select the proper timing to deliver such treatments, both in cancer and non-cancer patients." @default.
