- W1993156941 abstract "The role of endoscopic ultrasound (EUS) continues to expand. It has become the preferred mode of staging for pancreatic and esophageal malignancies. The expanded use of EUS extends to identifying extra-intestinal lymphomas specifically peripancreatic, ampullary, and periduodenal. These lesions often mimic pancreatic tumors on alternative imaging modalities. EUS provides an ideal mode for diagnosis and staging without surgical intervention. The importance of distinguishing between pancreatic malignancies and extra-intestinal lymphomas is crucial for patient treatment options and prognosis. We report 3 cases of presumed pancreatic masses found on CT scan (CT) referred for EUS diagnosis and staging. EUS revealed extra-intestinal masses in all 3 cases. Case 1: 66 year old female referred for EUS of a 5.4 cm pancreatic head mass identified on CT. EUS demonstrated a paraduodenal mass in the region of the pancreatic head. EUS with fine needle aspiration biopsy (FNAB) revealed follicular lymphoma, grade 2. Case 2: 47 year old male referred for EUS of an enlarged pancreatic head, body and tail with encasement of the splenic artery found on CT. EUS with FNAB revealed a peripancreatic large diffuse B cell lymphoma. Case 3: 74 year old male referred for EUS of a 4 cm lateral pancreatic head mass on CT. EUS with FNAB revealed peripancreatic Non-Hodgkin's B cell lymphoma. The importance of EUS in diagnosing extra-intestinal lymphomas continues to evolve. EUS coupled with FNAB affords the clinician an accurate minimally invasive diagnostic technique. EUS-FNAB sensitivity approaches almost 80% with a specificity of 100%. In our cases series all specimens were adequate for diagnosis. The presence of a cytopathologist to confirm specimen adequacy we feel improved the diagnostic yield of the EUS-FNAB in all cases. The 3 cases also highlight the limitation of CT imaging in distinguishing between pancreatic and peripancreatic tumors. Extra-intestinal lymphomas traditionally required laporatomy or CT-FNAB for diagnosis and staging. EUS-FNAB appears poised to replace this practice allowing for rapid accurate diagnosis upon which treatment can be based. The 3 patients in our case series all received chemotherapy with good clinical response 1 to 2 years from the time of initial diagnosis." @default.
