- W1993487332 abstract "RATIONALE: Mouse allergen (MA) and cockroach allergen (CA) exposure are associated with asthma morbidity, but their combined effects in sensitized and exposed individuals are unclear. METHODS: 144 children (5-17y) with asthma were skin prick tested (SPT) at baseline and had lung function, reversibility (>12% increase of FEV1 after bronchodilator), and health care utilization data collected at baseline, 3, 6, 9, and 12 months. Bed settled dust samples for quantification of Mus m 1 and Bla g 1 were collected at the same timepoints. +SPT was defined as a net wheal ≥3mm. Participants were stratified by exposure and sensitization as follows: 1) SPT- or not exposed to both MA and CA (SPT-/exp-), 2) cockroach SPT+ and exposed (cSPT+/cexp+), 3) mouse SPT+ and exposed (mSPT+/mexp+), 4) SPT+ and exposed to both (mcSPT+/mcexp+). Exposure was defined as bed MA>1.0 μg/g and bed CA>1U/g. All analyses were adjusted for age, gender, and serum total IgE. RESULTS: Participants were predominantly African American (91%), had public health insurance (85%), and had ≥1 +SPT (90%). The odds of health care utilization increased across the four groups (p=0.004): SPT-/exp- (ref); cSPT+/cexp+(OR[95%CI]: 0.88[0.47-1.66]); mSPT+/mexp+(1.70[1.04-2.77]); and mcSPT+/mcexp+(2.87[1.32-6.25]). The odds of reversibility also increased across groups (p=0.04): SPT-/exp- (ref); cSPT+/cexp+(0.50[0.21-1.23]); mSPT+/mexp+(1.65[0.96-2.84]); and mcSPT+/mcexp+(2.31[1.00-5.34]). Although not statistically significant, FEV1/FVC decreased across the four groups: SPT-/exp- (Mean±SD: 81.2±9.3); cSPT+/cexp+(82.1±9.7); mSPT+/mexp+(80.0±10.4); and mcSPT+/mcexp+(75.0±12.8). CONCLUSION: The combination of cockroach and mouse allergen exposures among mouse and cockroach sensitized children may lead to worse asthma control and greater morbidity than exposure to either allergen alone." @default.
