- W1993630892 abstract "To the Editor: We would like to congratulate Becker et al. for their exhaustive study, “Cerebro-Spinal Fluid Leak after Acoustic Neuroma Surgery: A Comparison of the Translabyrinthine, Middle Fossa and Retrosigmoid Approaches,”Otol Neurotol 2003;24:107–12) in which they have compared the rates of Cerebro-Spinal Fluid (CSF) leak in the commonly used approaches for the removal of acoustic neuroma with equal numbers of the patients for each one. In the paper, authors have also tried to compare the rates of CSF leak (0–30%) reported by different centers all over the world in the same surgical approaches. They conclude, “Reducing the incidence CSF leak associated with acoustic neuroma (AN) surgery has long been a goal of surgeon involved in the care of these tumors. Despite the efforts of many clever surgeons, the problem remains a significant cause of post operative morbidity”. They further mention, “Although currently used methods have reduced leakage rates to approximately 10 percent, this rate seems to have reached a plateau in recent decades”. We would like to draw the kind attention of the authors to one of the papers published by in your Journal that we have written (1). This paper deals with no CSF leak in 200 consecutive patients with AN who were operated on by the enlarged translabyrinthine approach from April, 1987 to September, 1998. In the paper, we have also described our technique of meticulous closure of all air cells with bone wax. The aditus is packed with pieces of periosteum medial and lateral to head of malleus (after removing the incus) to separate the middle ear from the mastoid. We are regularly following the same technique at our center, and untill now, we have operated on 500 patients with the same measures for the prevention of post-op leak, and we have encountered only 2 patients with post-op leak—of which, only 1 (0.2%) required reoperation. Wiet et al. (2) has also followed the same technique (closing the aditus with periosteum/bone wax) in 44 consecutive patients with AN treated with Translabyrinthine and had only 1 case (2.3%) of leak.
