We present our results with single stage repair of BXO-related distal strictures with circular BMG. METHODS Sixteen patients (age: 25 to 58 years) were operated between 2001 and 2010. Preoperative work-up included history, uroflowmetry and retrograde urethrography. Stricture was limited to fossa navicularis and external meatus (<1.5 cm.) in all cases. Through a circular perimeatal incision, distal strictured urethra was mobilized 0.5 cm. proximal to the healthy mucosa, then amputated for pathological examination (figure 1). A 4.5x2 cm. BMG was harvested from inner lower lip and its long side sutured circumferentially to the proximal urethra on a 24 Fr sound introduced from a transverse subcoronal incision (figure 2). Meatal reconstruction was completed after suturing contralateral BMG edge to the perimeatal incision (figure 2, insert). Foley catheter was removed within 2 weeks. Voiding symptoms, uroflowmetric parameters, cosmesis and recurrence were assessed at 1st, 3rd, 6th months, and yearly thereafter. RESULTS One patient developed early meatal stenosis at 1st month and regarded as failure. Peak urinary flow rate and voiding pattern improved significantly in the remaining patients (93%) (mean postoperative peak flow rate 22.4 vs. 8.9 ml/sec preoperatively). With a median follow-up of 24.6 months (range: 6 to 96) these patients maintained a cosmetic wide-caliber neo-meatus, and none developed recurrent strictures. CONCLUSIONS Glans penis has a good blood supply providing an efficient graft take.
