Studies have reported ED in association with the metabolic syndrome and as a precursor of both coronary artery and peripheral vascular disease in aging men. Body mass index (BMI) has been correlated with these conditions. The objective of this study was to evaluate the effect of BMI on penile duplex parameters in men with ED. METHODS We retrospectively reviewed the charts of 82 men presenting with ED that had failed PDE5 therapy. Men with known factors, chordee, history of pelvic trauma, current radiation/chemotherapy, or congenital abnormalities were excluded from review. BMI was calculated from height and weight measurements. Penile hemodynamics were assessed through penile duplex ultrasound following injection of a vasoactive agent. Peak systolic velocity (PSV) and end diastolic velocity (EDV) were measured while the resistive index (RI) was calculated from these values. We divided our patient population into three groups: Group 1: BMI less than 25 (Normal), Group 2: BMI between 25 and 29.9 (Overweight), Group 3: BMI 30 or greater (Obese). Unpaired, two-tailed T-tests were performed to assess variability of PSV, EDV, and RI across men in Groups 1, 2 and 3. A linear regression model was performed to assess trends. RESULTS We did not find a significant difference in the PSV of men in Group 1 when compared to the PSV in men in Group 2 and Group 3. However, when we combined Group 1 and Group 2 men together, and compared them to Group 3, we found a significant difference in PSV (p<0.05). When we compared the PSV in Group 2 to Group 3, we also found a statistically significant difference in PSV (p<0.05). A linear regression analysis demonstrated a significant inverse relationship between PSV and BMI in men presenting with ED (p<0.05). CONCLUSIONS Our results demonstrate that a BMI greater than 30 predicts significant impairment in peak systolic velocity.
