- W1993879433 abstract "Background: Causes of missing lesions during colonoscopy include lesions hidden behind folds and subtle lesions in the visual field that cannot be readily seen (e.g. flat and depressed lesions). NBI with blue light provides increased mucosal detail in the endoscopic image. Whether NBI can improve detection of adenomas is uncertain. Aim: Perform a randomized controlled trial to determine if more adenomas are found using NBI compared to WL. Methods: Olympus HD 180 series colonoscopes with push button switch from WL to NBI were used by a single experienced endoscopist. Thus far, the study has included 70 of a planned 434 patients. The scope is inserted to the cecum using WL and patients are then randomized to undergo examination withdrawal with WL vs NBI in a 1:1 ratio. Indications for the exam included: screening, surveillance, abdominal pain, or anemia. Patients were age ≥50 and did not have prior colorectal resection, inflammatory bowel disease or known polyp syndromes. A stopwatch recorded withdrawal times and excluded time for polyp removal, suctioning and cleaning. Withdrawal time was targeted to 8 minutes. Results: There was no difference in gender (p = 0.34) or age (p = 0.89) between the arms. 34 patients in the WL arm and 36 in the NBI arm are fully evaluated. The number of adenomas found in the WL group (74; 84% ≤ 5 mm) was higher than the NBI group (48; 81% ≤ 5 mm) (p = 0.047). The number of flat (JRSC type 2) adenomas was 33 in the WL group and 21 in the NBI group (p = 0.37). The percent of patients with ≥1 adenoma with WL was 71% (24/34) and NBI was 47% (17/36). The examination times during withdrawal did not differ (p = 0.23): WL mean 8.1 min (SD 1.5, range 5.6-11.0 min) and NBI mean 8.5 min (SD 1.5, range 5.6-12.8 min. Conclusions: Preliminary results of this randomized trial do not demonstrate that examination with NBI detects more adenomas than WL during routine colonoscopy in a US population. The adenoma detection rate per colonoscopy in both arms exceeds all previous reports in routine colonoscopy including the chromoendoscopy arms of the 2 previous randomized trials of chromoendoscopy vs WL (J. Brooker Gastrointestinal Endoscopy 2002 56(3):333-8; Hurlstone 2004 Gut. 53(3):376-80). This might be secondary to the HD function of the colonoscopes." @default.
