Sie ist das seelische Bindeglied aller Untersuchungsbefunde und ermöglicht allein die individuelle Beurteilung des Einzelfalles. Sie schafft die subjektiv-objektive Ausgangsbasis, sie vermittelt den persönlichen Kontakt zum Patienten, erschließt die von diesem selbst reproduzierten Zusammenhänge und ermöglicht einen aufschlußreichen Einblick in seine Persönlichkeitsstruktur. Obwohl lediglich auf Erinnerung, Deutung und Kombination fußend, erhält die Vorgeschichte juristische Beweiskraft, eine Tatsache, deren Bedeutung man sich bei ihrer Erhebung stets vor Augen halten sollte. Der Arzt hat somit eine doppelte Verpflichtung zur sorgfältigen Registrierung der Vorgeschichte. Sie besteht einerseits gegenüber dem Patienten, der ein Recht darauf hat, alle Vorgänge, auf die er sich besinnt, festgehalten und bewertet zu sehen; sie ist andererseits notwendig zur Vermeidung von Fehldiagnosen. Man vergegenwärtige sich daher, daß man nie mehr die Möglichkeit einer ungefärbten Darstellung haben wird als bei der sorgfältigen und jede Suggestion vermeidenden Erstexploration, die so früh wie möglich durchgeführt werden sollte. Nur zu diesem Zeitpunkt ist die Darstellung noch frei von Spekulationen und Tendenzen, von neurotischen Überlagerungen bzw. fixierten Vorstellungen oder Redewendungen. Jede spätere Befragung dagegen ist beeinflußt durch nachträgliche Überlegungen, Überlagerung mit Schilderungen Dritter, durch bewußte oder unbewußte Erinnerungstäuschungen, durch die Stereotypie immer wiederholter Darstellungen. durch Temperament, Phantasie und Geltungsbedürfnis. Diese Tatsachen muß man sich vergegenwärtigen, wenn man der Sorgfaltspflicht genügen und nicht auf ein wichtiges und unwiederbringliches Material verzichten will." @default.
