- W1994231569 abstract "Although cardiovascular disease remains the main cause of disability and premature death worldwide, the past decade has seen a dramatic fall in the morbidity and mortality attributable to heart disease in Western countries such as the UK and USA. Partly this can be ascribed to improvements in cardiovascular disease prevention, with more rigorous screening for, and more vigorous treatment of, risk factors such as hypertension and dyslipidaemia, coupled with a significant decline in smoking rates. In addition, however, great strides have occurred in interventional cardiology, and especially in percutaneous coronary interventions, to such an extent that these can now be applied to large numbers of patients who in the past would have needed coronary bypass surgery; indeed, cardiac surgery rates have declined dramatically as a result. The advances in interventional cardiology have been both technical and pharmacological. Technical improvements have meant that lesions can now be tackled which were either inaccessible or too risky in the past. Pharmacological improvements have resulted in better long-term coronary patency rates and freedom from clinical disease. It is this last aspect, the pharmacology relevant to interventional cardiology, which is tackled by this book.It is certainly the case that the pharmacological knowledge needed by practising interventional cardiologists has increased almost exponentially in the last 10–15 years, in line with the increased array of effective therapies available. As a consequence, it is certainly an opportune time for a book such as this to appear. It deals in a very comprehensive manner with the therapeutic agents available to the practising interventional cardiologist and, to its credit, explains the basic pharmacology of these agents in appropriate (but not excessive) detail as well as their clinical application. In its later parts, it also covers noncoronary interventions, and goes into some technical detail here, but also provides relevant and important information and guidance as regards adjunctive pharmacotherapies. The book also covers emerging and future therapeutic possibilities, their possible application as well as their drawbacks and limitations: these include the use of antisense oligonucleotides, other gene therapies, and cell therapies including therapeutic angiogenesis.Overall, this book is well written, by a collection of authors who are well respected in their particular fields. Of course, there are many cardiovascular pharmacology texts on the market, but this in my view is the best one currently available that is specifically aimed at the interventional cardiologist. As well as being eminently readable (despite its size – nearly 700 pages), it is an authoritative textbook in its field.The difficulty with any textbook in this area, however, is the rapidity with which the subject is advancing, so that any such text is out of date almost as soon as it is published. This is, of course, not the fault of the authors or editors, and it underlines the impressive rate of progress that is being made in the treatment of heart disease. Long may this continue." @default.
