• W1995305945 abstract "Mediante la presentacion de un caso clinico de acromegalia se repasan los tratamientos disponibles para esta enfermedad, tanto los primarios (aplicados tras el diagnostico inicial), como los secundarios (aplicados tras un primer tratamiento ineficaz o insuficiente). Se pretende que la sesion sea interactiva mediante preguntas para las que existan diferentes respuestas, y cada una de ellas se comenta en la discusion posterior. Se repasan los tratamientos medicos (analogos de somatostatina, farmacos dopaminergicos, inhibidores del receptor de hormona de crecimiento), el tratamiento neuroquirurgico y la indicacion de un abordaje transesfenoidal o transcraneal y las diversas formas de radioterapia (convencional, radiocirugia o gamma-knife y radioterapia esterotaxica fraccionada). Se discuten, asimismo, los criterios de curacion en cuanto a las concentraciones circulantes de hormona de crecimiento, factor de crecimiento similar a la insulina tipo I y como estas han cambiado a lo largo de los anos, a la vista de la diferente morbimortalidad en los pacientes con acromegalia considerada activa. Por ultimo, se discute la importancia de remitir al paciente a un servicio neuroquirurgico con experiencia, dada la enorme diferencia que existe en los resultados en funcion de esta. The presentation of a case of acromegaly will be used to review the available treatments for this disease, both primary (to be applied after initial diagnosis), and secondary (to be applied if the initial treatment is unsuccessful or insufficient). The session will be interactive through questions to which there are several answers, each of which will be commented on in the discussion that will follow. Medical treatments will be reviewed (somatostatin analogs, dopaminergic drugs, growth hormone receptor inhibitors), as well as neurosurgical treatments and the indications for transsphenoidal and transcranial approaches, and the different forms of radiotherapy (conventional, radiosurgery or gamma-knife and fractionated stereotaxic radiotherapy). We will also discuss the criteria for cure concerning circulating growth hormone and insulin-like growth factor-1 and how these have changed over the years, in view of the different morbidity and mortality in patients with active acromegaly. Finally, the importance of referring patients to a neurosurgery department with experience in this disease will be discussed, given the enormous difference in the results obtained according to this factor." @default.
