During this time changes in the dose of DPH were made in some patients in order to increase control of seizures or decrease side-effects: in 27 patients the dose was increased, in nine it was decreased and in 12 the dose remained unchanged. It was found that the severity of gingival hyperplasia varied directly with both the dose and serum level of the drug. RÉSUMÉ Hyperplasie gingivale provoquée par la diphenylhydantoïne: réponses aux modifications de dose Les variations de l'hyperplasie gingivale ont étéétudiées chez 48 sujets traités pour épilepsie par diphenylhydantoïne (DPH) pendant deux ans. Durant cette période, des modifications de la dose de DPH ont été faites chez quelques sujets pour accroitre le contrôle des crises ou diminuer les effets secondares: les doses ont été augmentées chez 27 sujets, diminuées chez neuf, inchangées chez douze. Il a été observé que l'intensité de l'hyperplasie gingivale variait directement en fonction de la dose et du taux sérique du médicament. ZUSAMMENFASSUNG Die Diphenylhydantoin-induzierte Gingivahyperplaisie: Ihr Ansprechen auf Veränderungen in der Dosierung Über einen Zeitraum von zwei Jahren wurden die Veränderungen der Gingivahyperplasie bei 48 Patienten, die wegen einer Epilepsie mit Diphenylhydantoin (DPH) behandelt waren, untersucht. Während dieser Zeit wurden bei einigen Patienten Änderungen in der DPH-Dosierung vorgenommen, um eine bessere Kontrolle der Krampfanfälle zu erzielen oder eine Reduzierung der Nebenwirkungen: bei 27 Patienten wurde die Dosis gesteigert, bei 9 Patienten vermindert und bei 12 Patienten blieb sie unverändert. Man fand, daß die Schwere der Gingivahyperplasie direkt sowohl mit der Dosis wie auch mit dem Serum-spiegel variiert. RÉSUMÉN Hiperplasia gingival inducida por difenilhidantoina: Su respuesta a los cambios en las dósis del fármaco Se estudiaron los cambios en la hiperplasia gingival en 48 pacientes tratados por epilepsia con difenilhidantoína (DPH) durante un periodo de más de dos años. Durante este tiempo se hicieron cambios en la dosis de DPH en algunos pacientes con el objeto de aumentar el control de las convulsiones o disminuir los efectos secundarios: en 27 pacientes la dósis se aumentó, en 9 se redujo y en 12 se mantuvo igual.
