- W1995580066 abstract "Description of case(s): A patient presented with painless vaginal bleeding at 7 weeks amenorrhoea. Ultrasound examination showed the presence of an intracervical gestational sac containing a viable fetus of corresponding fetal parameter, closed internal cervical os, trophoblastic invasion to the endocervical tissue, empty uterine cavity with decidual reaction, hourglass uterine shape, and intracervical peritrophoblastic blood flow, satisfying the previously described criteria. In addition, the vascular supply of the trophoblastic invasion was mapped out with colour and power Doppler examination and was delineated to the descending branch of the right uterine artery. The management options and the pros and cons were discussed with the patient. She decided for medical treatment. The cervical pregnancy was aborted vaginally with the use of systemic methotrexate and vaginal misoprostone while selective right uterine artery ligation and selective right uterine artery embolisation remained the other options. Proof of diagnosis: Ultrasound examination showed the presence of a gestational sac in the anterior wall of the cervix. A fetus of crown rump length corresponding to 7 weeks was present. Fetal heart pulsation was present. Trophoblastic invasion was present around the sac. The uterus was normal in size with thickened endometrium. Doppler examination showed that the descending branch of the right uterine artery branched out to encircle the gestational sac. There was no supply from the left uterine artery. Relevance: The use of ultrasound and Doppler examination permits the confident use of certain treatment options in cervical pregnancy. It enables better counseling on the management of cervical pregnancy and possibly better choice of treatment options." @default.
