- W1995736174 abstract "Purpose/Objective: To evaluate the outcome of patients treated with Gamma Knife radiosurgery for trigeminal neuralgia.Materials/Methods: The HealthSouth-University of Alabama Gamma Knife unit treated 163 patients with facial pain between 10/95-8/01. 113 patients were classified as having typical trigeminal neuralgia. All patients had failed prior medical and or surgical management. This report includes the outcome of 104 patients who have a minimum follow-up of 6 months. Thirty-six patients were treated with 35 Gy to the 50% isoline and 68 patients were treated with 40 Gy to 50% isoline. In most instances a single 4mm collimator was positioned to target the involved trigeminal nerve adjacent to the exit from the pons. All patients were monitored routinely with follow-up visits or telephone interviews.Results: The median follow up for the entire group is 28 months with a range of 6–72 months. Seventy-two percent of patients achieved a beneficial result defined as an excellent response (pain free on no meds) or a good response (50–90% improvement, still on meds). A 52% response rate were achieved in patients treated with 35 Gy and 81% response in patients treated with 40 Gy. Seventeen patients developed new paresthesias. Fifteen of these patients received 40 Gy. Two patients received 35 Gy and one of the two patients had transient symptoms. Four of 17 patients developed somewhat bothersome or very bothersome paresthesias.Conclusions: Trigeminal neuralgia is often extremely debilitating and can require management with multiple medications or invasive surgery. Radiosurgery provides an effective treatment alternative. Sixteen percent of our patients developed facial paresthesias with 4 of the patients noting bothersome symptoms.
