- W1995864057 abstract "Recent studies have shown SBRT to be an effective treatment for selected patients with node-negative non–small cell lung cancer. Some data exist that suggest an increase in toxicity for central lesions, defined as those within 2 cm of the proximal tracheobronchial tree. We retrospectively evaluated patients treated with SBRT at our institution to determine whether central location of tumor predicted for inferior survival or increased toxicity. The records of the Indiana University Department of Radiation Oncology and Roudebush VAMC were screened for patients receiving SBRT for Stage I/II NSCLC. Patients were divided by central (n=31) or peripheral (n=249) lesion sites. Mann-Whitney rank-sum tests were performed comparing central and peripheral samples. chi-square tests were used to evaluate toxicity, local failure, and survival. Median follow-up was 15.3 months (range, 0.2–89.3 months) and was not different between groups. Between groups, there was no statistically significant difference in GTV or PTV size, dose per fraction, total dose, or number of beams used. Central lesions were treated with significantly more fractions (p = 0.03). There was no difference between central and peripheral cohorts in regard to local failure (chi-square p = 0.32) or overall survival (chi-square p = 0.69). There was also no difference in patients experiencing ≥ Grade 2 toxicity (chi-square p = 0.13); however, patients with central lesions had more ≥ Grade 3 toxicity (chi-square p = 0.017). In patients with central lesions, those treated with four or more fractions had fewer ≥ Grade 3 toxicities (1/13, 7.7%) than those treated with three fractions (5/19, 26.3%), but this difference did not reach statistical significance (chi-square p = 0.36). Local control and overall survival were similar in patients with central and peripheral lesions. Grade 3 and higher toxicities occurred more frequently in central tumors. Further exploration into the effect of increased fractionation on toxicity is warranted." @default.
