- W1996292012 abstract "Abstract The prevalence of child sexual abuse (CSA) is approximately 12–38% of women and 8% of men. Therapy involves patients working through feelings and experiences by talking through their story gradually. Progress is shown by movement through cycles of grief, towards increasing acceptance of the reality of the abuse and its effects. The work focuses on evolving from ‘victim’ towards ‘survivor’ and beginning to learn to ‘thrive’ in the present. Progress is helped by allowing the patient to take control of their therapy, working at a pace set by them, and working with what the patient brings. Patients may fear that the therapist cannot truly understand, or will be damaged by the retelling of their story. Trust can be built by a stance of belief towards the patient, demonstrating therapist reliability (e.g. firm boundaries on session arrangements) and ensuring the safety of the therapeutic space (e.g. an assurance of no physical contact in sessions). Sexual abuse thrives through deception, secrecy and collusion. For patients who fear re-experiencing the secretive, unboundaried experience of abuse, a climate of open enquiry in a non-judgmental atmosphere can be reassuring. Group membership reduces isolation and stigmatization. Being believed and listened to by peers enables self-acceptance and seeing experiences mirrored in other survivors enables self-reflection. Denial, avoidance and maladaptive coping strategies can be effectively challenged by peers and groups can provide role models, coping strategies and the opportunity for information-sharing." @default.
