With hypoglycemia, EEG tracings in normal individuals showed diffuse background slowing, whereas 7 of 22 patients developed focal temporal changes, including focal spike and focal slow wave activation. The development of focal changes correlated well with clinical data concerning underlying focal pathology; focal abnormalities were not evoked in patients with multifocal disease. Hypoglycemic activation of the EEG may be a useful technique for predicting the presence of pathology in patients considered for anterior temporal lobectomy. L'EEG a été enregistré chez 22 patients avec épilepsie partielle complexe réfractaire à la médication et subissant un examen pré-opératoire et chez 11 témoins d'âge correspondant, alors qu'ils étaient soumis à hypoglycémie modérée afin de déterminer si les modifications ainsi produites permettaient de prédire une pathologie sousjacente. Cinq patients ont eu un EEG de jeûne avec anomalies focalisées, qui n'étaient pas apparues au cours du stade d'alimentation normale. Les tracés EEG des sujets témoins ont montré un ralentissement diffus du tracé de fond, alors que chez 7 des 22 patients, des modifications temporales focalisées se sont développées, avec dents de scie et ondes lentes. L'apparition de modifications focalisées était bien corrélée avec les données cliniques sur la pathologie focale sous-jacente; des anomalies focalisées ne sont pas apparues chez des patients avec une pathologie multifocale. L'activation de l'EEG par hypoglycémie pourrait être utile pour révéler une pathologie chez des patients pris en considération pour une lobectomie temporale." @default.
