- W1996543108 abstract "We have previously employed an event-related potential (ERP) paradigm, in which semantically congruous or incongruous target words are repeated to modulate the P600 and N400 potentials, respectively. These studies showed that ERP word repetition effects are very sensitive to early Alzheimer's disease (AD). We have now adapted this paradigm for functional MRI (fMRI) to compare word repetition effects in mild AD with normal elderly. Fifteen patients with mild AD (mean age = 73) and 15 older normal controls (NC; mean age=71) underwent 1.5T fMRI while they performed a semantic category decision task. Event-related fMRI analyses were conducted with AFNI. Behavioral data showed larger “priming” effects (i.e., reaction time difference for New -Old words) in NC than AD for congruous, but not incongruous, trials. fMRI analyses showed robust between-groups differences in the BOLD response patterns to congruous words. In NC, the New Old contrast demonstrated larger BOLD responses in much of the left hemisphere (including several regions which are putative P600 generators: parahippocampal, cingulate; perirhinal, middle temporal and inferior frontal gyri), while the Old New contrast showed much less widespread activation, appearing mainly in the right parietal cortex. This response pattern resembles the hemispheric encoding/retrieval asymmetry (HERA) described in several prior neuroimaging studies of memory. In contrast, relatively few regions had significant New Old BOLD responses in mild AD. Unlike NC, these activations were mainly found in the right hemisphere (e.g., right inferior parietal lobule). In AD, the Old New contrast did not demonstrate the right-hemisphere predominant activation observed in NC. The spatial extent of significant New Old BOLD responses in the left medial temporal lobe (MTL) correlated with subsequent cued recall (r = .61, p < 0.001). Decreased congruous word repetition effects were observed with fMRI in mild AD, particularly in left association cortex, paralimbic, and MTL structures. This suggests that a distributed network of P600-generators in the left hemisphere, which normally has New Old BOLD responses to congruous words, is important for the successful encoding of verbal stimuli. This network appears highly dysfunctional in mild AD." @default.
