Nous avons noté un gain de un à trois points du score d’Aschworth modifié. Les résultats obtenus permettaient une orientation optimale de la conduite thérapeutique ultérieure. La toxine botulinique était indiquée chez 90 patients mais n’était réalisée que dans 22 cas. Dix interventions orthopédiques étaient indiquées d’emblée. Une neurotomie du nerf tibial après bloc diagnostique a été réalisée dans deux cas. Les résultats de la neurotomie 1 an après l’intervention ont été identiques à ceux observés par le bloc. Les blocs anesthésiques sont d’un intérêt incontournable dans l’arsenal thérapeutique de la spasticité focale. Ils nous permettent d’avoir une approche diagnostique et pronostique avant d’aborder un traitement coûteux et/ou lourd pour les patients. To specify the interest of anesthetic blocks in therapeutic decision and management of spasticity in our country where conditions are limited. A retrospective study including 100 patients. A clinical evaluation was made before and after the block with a determination of spasticity, a measurement of articular amplitudes, a quotation of muscular strength and a functional evaluation. We did 146 blocks for 100 patients. We carried out one hundred and forty-six blocks for hundred patients. The injected sites were dominated by the posterior tibial nerve (63 blocks), the median nerve (34 blocks) and the obturator nerve (22 blocks). We noted a gain from one to three points in the score Aschworth changed. The results allow optimal orientation of the subsequent therapeutic management. Botulinum toxin was indicated in 90 patients but was performed in 22 cases. Ten orthopedic surgical procedures were indicated. Neurotomy of the tibial nerve after diagnostic block was performed in two cases. The results of the neurotomy 1 year after surgery were similar to that observed by the block. Anesthetic blocks are a key interest in the therapeutic arsenal of focal spasticity. They allow us to have diagnostic and prognostic approach before tackling an expensive and/or difficult treatment for patients." @default.
