Conventional teaching has stated that significant preoperative hypertension is reason to delay elective surgery pending better medical control. Proponents of this approach point to reports of exaggerated intraoperative hemodynamic variation, relative intravascular volume depletion, and increased risk of cardiovascular and cerebral morbidity. During cardiac surgery, intraoperative control of acute hypertension is essential to reduce bleeding from suture lines, improve visualization and operating conditions, and reduce myocardial wall tension and subsequent oxygen consumption. Data to support either the preoperative control of moderate levels of hypertension before surgery or the correlation between intraoperative levels of blood pressure and cardiovascular morbidity are lacking. Although control of extreme levels of hypertension may be beneficial, the effect of intervention at lesser levels is less certain. Questions remain: What levels of hypertension require preoperative treatment? What should the target blood pressure be? This article examines what is known and tries to answer these questions." @default.
