• W1997325859 abstract "Background Tracheobronchial adenoid cystic carcinoma (ACC) is a rare and potentially malignant subtype of salivary gland mucoepidermoid tumors. There is consensus regarding definite treatment of this tumor. Our study is a retrospective review of therapeutic methods used in patients with tracheobronchial ACC in our 7 years of experience and arrives at practical recommendations. Materials and Methods Medical records of 10 patients with tracheobronchial ACC who were referred to our institution between the years 1999 and 2006 were studied. All patients received neodymium yttrium aluminum garnet (Nd:YAG) laser photoresection as a palliative treatment. The procedures were performed under general anesthesia via rigid bronchoscope. Patients with distant metastasis received systemic chemotherapy in addition to the palliative treatment. Result Three of patients were smokers. Symptoms at the time of presentation included dyspnea (n=6), hemoptysis (n=5), cough (n=4), and hoarseness (n=1). Patients were in 3 histopathologic grading according to R.O. Anderson Cancer Center and Batsakis. Five had grade I, 4 had grade II, and a patient had grade III of histopathology grading. After 9 sessions of laser resection, patients' symptoms were dyspnea (n=3), hemoptysis (n=1), cough (n=2), and hoarseness (n=2). Complications during palliative procedures included hypoxemia (n=4) and bleeding (n=2). Distant metastasis was seen in 40% of patients with grade I, 50% of patients with grade II, and 100% of patient with grade III, during 3 years of follow-up. Seven patients died because of the progression of the tumor or respiratory failure. Three patients with central airway lesion are still alive with mild dyspnea and cough, but without any metastatic spread, 2 of them had grade I lesions. Conclusions It seems that tracheobronchial ACC is more common among men and may not be related to smoking. We suggest that endoscopic treatment with endobronchial Nd:YAG laser and stent insertion play an important role in palliation of patients with tracheobronchial ACC. These endoscopic procedures palliate symptoms and complications, such as obstructive pneumonia and hemoptysis. It is difficult to find the correlation between the tumoral histopathologic grading and patient survival in our limited study. It seems that surgical resection should be the treatment of choice in patients with localized tumor without distant metastasis." @default.
