- W2000009621 abstract "傍十二指腸ヘルニアは，Treitz靱帯周囲の腹膜窩に腸管が嵌入する比較的稀な内ヘルニアである．今回，われわれはMDCTにより術前診断しえた左傍十二指腸ヘルニアの2例を経験し，それらに対して腹腔鏡手術を施行したので報告する．症例1は64歳，男性．MDCT検査にて，左上腹部に嚢状の拡張した腸管を認め，その腹側には下腸間膜静脈（IMV）を認めた．腹腔鏡下に左傍十二指腸窩に嵌入した小腸を整復し，ヘルニア門を閉鎖する腹腔鏡下手術を行った．症例2は40歳，男性．MDCT検査では，膵尾部下方に小腸が一塊となって認め，その腹側にIMVの走行が確認された．手術は，上十二指腸窩に大きなヘルニア門を認め，小開腹を追加した腹腔鏡補助下手術を施行した．以上より，MDCT検査は傍十二指腸窩ヘルニアの診断に優れ，腹腔鏡手術は本症例に対する有効な治療法と考える．" @default.
