The spread of HIV infection into the heterosexual population in the United States, Europe, and Australia and its long-term heterosexual presence in the developing world have led to increased scientific and clinical attention to the role of HIV infection in pregnancy. As of 1993, a cumulative total of about 1 million children have contracted HIV infection, most commonly through vertical transmission (i.e., transmission from mother to child).84 It is estimated that by the year 2000, 6 million pregnant women and 5 to 10 million children will have HIV infection.76 HIV infection has a profound effect on the medical community and society at large. The burden of caring for a large group of affected individuals has led to questions about basic issues in health care, including governmental support of basic sciences, the rapid development and governmental approval of effective therapies, access to medical care, patient confidentiality, and patient discrimination. The problems are compounded in the case of HIV and pregnancy because both mother and child have to be considered. Conflicting interests between the mother and child have to be resolved, such as conflicts regarding mandatory testing for HIV infection, in which the rights of the unborn child to receive antiretroviral therapy to reduce the risk of contracting HIV infection (see later) must be weighed against the right to confidentiality of the mother, including the right to refuse tests for HIV infection.34, 86" @default.
