- W2000247028 abstract "In today’s Lancet Theonest Mutabingwa in Tanzania and Patrice Piola in Uganda and their respective colleagues report effectiveness trials with different antimalarial combinations to treat patients with uncomplicated Plasmodium falciparum malaria. Thereby they aimed to mimic the real world of malaria treatment which in this case mainly means unsupervised medication taken at home. As in other infectious diseases such as tuberculosis and AIDS it has become fashionable and standard and it is recommended by WHO to use combination regimens for treating P falciparum malaria especially artemisinin containing regimens. For some people the recommendation of artemisinin-containing regimens seemingly has become a religion. Therefore it is important and refreshing to see that not everybody is putting all eggs in one basket and to look at other than the currently well-working artemisinin-based combinations to have alternatives if needed. Examples developed and sponsored by the pharmaceutical industry of such combinations are atovaquone-proguanil with its price problems and chlorproguanil-dapsone with haematological safety concerns which will certainly not be resolved by the addition of artesunate. Other examples of ad-hoc combinations are quinine plus clindamycin quinine plus sulfadoxine-pyrimethamine fosmidomycin plus clindamycin and amodiaquine plus sulfadoxine-pyrimethamine. Amodiaquine plus sulfadoxine-pyrimethamine was proposed as an interim therapy after chloroquine amodiaquine or sulfadoxine-pyrimethamine could not be used successfully any longer in most African countries (because of widespread drug-resistance) and before more effective cheap antimalarial combinations would become available. Among the artemisinin-based combinations artesunate-amodiaquine and artemether-lumefantrine are the most advanced best developed marketed and currently most advocated for treating African children with uncomplicated P falciparum malaria.
