- W2000315424 abstract "Plasma sphigolipids have been shown to predict cognitive impairment and hippocampal volume loss, but there is little research in subjects with Alzheimer's disease (AD). Further, only ceramide and sphingomyelin (SM) species with a sphingosine (C18:0) backbone have been considered. In this study we compared plasma ceramide and SM species with sphingosine (C18:1) or sphinganine (C18:0; a dihydroxy derivative of sphingosine) backbones as predictors of clinical progression among AD patients. Probable AD patients (n = 114) were followed annually in the Alzheimer's Disease and Memory Disorders Center at Baylor College of Medicine. Plasma sphingolipids were assessed using ESI/MS/MS in all patient samples collected at baseline. Cross-sectional and longitudinal regression analyses examined baseline plasma lipids in cross-sectional and one-year longitudinal change on the Mini-Mental State Exam (MMSE), Alzheimer's Disease Assessment Scale-Cognitive Subscale (ADAS-Cog), and Clinical Dementia Rating-Sum of Boxes (CDR-Sum). There were no cross-sectional associations between levels of plasma sphingolipids and psychometric scores. In longitudinal analyses, higher total plasma SM (C16:0-C24:1 combined) was associated with less progression on the MMSE and ADAS-Cog: the strength of these associations was dependent on the structure of the backbone. Compared to the lowest tertile, the highest tertile of SMs with sphinganine backbones (dihydrosphingomyelins) declined 1.5 points (p = 0.045) less on the MMSE and increased 4.1 fewer points (p = 0.003) on the ADAS-Cog. In contrast, the highest tertile of SM with sphingosine backbones increased 2.9 points (p = 0.034) less on the ADAS-Cog compared to the lowest tertile and there was no association with the MMSE. Plasma ceramides did not predict decline on any test. These results suggest that high levels of SMs, but not ceramides, dose-dependently predict slower one-year decline on cognitive measures. Total plasma dihydrosphingomyelin appears to be a better predictor for cognitive progression and could provide a new treatment target." @default.
