- W2000343712 abstract "Individual target motion is a major problem in high precision radiation therapy techniques. Four-dimensional list mode PET/CT can solve this problem for PET visible lesions by valid detection of movements. This study evaluates our clinical experience using 4D list mode- based target delineation in free breathing stereotactic body radiation therapy (SBRT) of liver tumors. Eighty-five patients were enrolled. They suffered from metastases (n = 76; one to four lesions), Klatskin tumor (n = 7) or hepatocellular cancer (n = 2). Planning procedure included free breathing 4D list mode PET/CT and MRI. During scans and treatment, patients were positioned in a Body System. PET/CT data were acquired on a list mode - capable PET/CT scanner. PTV included gross tumor volume, 2 mm set-up margin, and safety margins based on list mode detected motion. Normally, the patients underwent SBRT with 3 x 12.5 Gy to the 65 % enclosing isodose. If stomach or small bowel had to be included in PTV, the prescribed dose was 5 x 7.0 Gy. Dose was delivered by tomotherapy. Clinical history, laboratory findings, early and late toxicity scores (CTCAE v. 3.0), PET/CT, and MRI were gathered at the 6-week follow-up visit and then every 3 months. All lesions were visible in PET and motion was detected by list mode PET/CT in all cases. The patients tolerated planning procedure and SBRT very well. No early or late toxicity ≥2 was reported. Local control is 98.8 % (3 to 25 month's observation time, mean 13.1). High individualization allows valid planning and application procedure. The constituted high tumor control rate and low toxicity scores demonstrate excellent applicability of 4D-list mode based target delineation in free breathing SBRT for patients suffered from liver tumors." @default.
