- W2000358710 abstract "Peptic esophagitis results from persistent gastroesophageal reflux of acid gastric juice. A variety of operative procedures have been employed for correction of reflux. Laboratory evaluation of postoperative reflux is essential for objective assessment of operative therapy. Esophagoscopy with biopsy is the most sensitive objective measure of reflux esophagitis, but patient acceptance has been a limiting factor. The pH reflux test and acid clearing test are the most sensitive objective measures which are easily performed. Standard pH probe withdrawal reflux tests have a one third incidence of false-positive results and, thus, have limited predictive value. However, a negative pH reflux test result has excellent prognostic value and correlates well with clinical success. A modification of the standard pH reflux test consisting of antegrade passage of a pH detector more accurately determines the presence or absence of gastroesophageal reflux. Crural repair for correction of gastroesophageal reflux is dependent on an intact repair. The incidence of both anatomic recurrence of sliding hiatal hernia and clinical recurrence of esophagitis is discouragingly high. Fundoplication is effective in preventing reflux, but the morbidity associated with valvuloplasty is distressing." @default.
