AD-like Aβ pathology is a common comorbidity in patients with normal pressure hydrocephalus (NPH).In this reportwedemonstratethe effectiveness of quantitative [18 F]flutemetamol PET for detection of brain amyloidosis in patients with NPH. Fifty patients with NPH needing ventriculo-peritoneal shunting or intracranial pressure monitoring underwent [18 F]flutemetamol PET before (N=28) or after (N=22) surgery. [18 F]flutemetamol uptake was quantified by standard uptake value ratio (SUVR) for 3 brain 'regions' in relation to either cerebellum or pons reference region (REF): cerebral cortex surrounding the biopsy, contralateral homolog, and calculated composite brain measure. Brain biopsy Aβ pathology (standard of truth [SoT]) was measured using Bielschowsky silver and thioflavinS stains (plaque scores), percent area of gray matter positive for monoclonal antibody to Aβ (4G8), and overall pathology impression. Of the 24possible pairs (4 pathology reference standards x 3 brain regions x 2 imaging references) of endpoints, the composite-cerebellum SUVR/overall pathology SoT pair matched best, having the highest Youden index (1.00), receiver-operator curve area under the curve (1.00), sensitivity (1.00), specificity (1.00), and sum (sensitivity + specificity) for the pooled data, and the retrospective and prospective studies separately (2.00, for all 3). .The composite SUVR had a 100% positive predictive value (both REFs) for the overall pathology diagnosis. All SUVRs (ipsilateral, contralateral, and composite using both cerebellum and pons REF) had a 100% negative predictive value for the Bielschowsky silver result. Bielschowsky silver and overall pathologies showed the strongest associations with quantitative [18 F]flutemetamol PET imaging results." @default.
