- W2000401417 abstract "Objective: In-vitro fertilization (IVF) with intracytoplasmic sperm injection (ICSI) has recently been offered to HIV-discordant couples where the man is seropositve and the woman seronegative to achieve pregnancy while minimizing the risk of HIV transmission. Unfortunately, not all couples are IVF candidates due to factors inherent to the female partner. The purpose of this study was to assess the utility of using donor oocytes in discordant couples who have previously failed conventional IVF therapy. Methods: 14 of 56 (25%) couples enrolled in IVF-ICSI were unable to conceive due to various factors, such as advanced reproductive age (n = 9) or poor response to ovarian stimulation (n = 4). Of these, five HIV-serodiscordant couples chose to pursue oocyte donation. Husbands aged 40.6 ± 6.1 (mean ± standard deviation) (range 35–49) years were asymptomatic HIV-seropositive with viral loads of 52.4 ± 91.8 (range 0–212) copies/ml and CD4 counts of 503.6 ± 350.6 (range 50–1000) per mm3. All were previously unable to conceive with standard IVF-ICSI with 2.75 ± 1.7 (range 1–5) prior cycles. One patient also previously failed 3 cycles of intrauterine insemination with washed sperm. Female partners were 40.4 ± 3.4 (range 36–45) years, with day 2 FSH values of 10.0 ± 5.2 (range 5.5–17.5). Recipients were matched anonymously to oocyte donors, who underwent IVF-ICSI with ICSI performed on sperm prepared by a modified wash swim-up technique described by Semprini. Results: Five HIV-serodiscordant couples underwent 6 oocyte donation retrieval cycles yielding 16.8 ± 9.5 (range 11–34) oocytes; 8.7 ± 4.5 (range 4–17) embryos; and 1.8 ± 2.2 (range 0–5) cryopreserved embryos. The ICSI fertilization rate was 0.53 ± 0.11 (range 0.36–0.64). Two of five couples conceived after one oocyte donation cycle (one singleton pregnancy and an ongoing triplet pregnancy). Two of the three couples who failed the first oocyte donation cycle underwent an FET cycle with available cryopreserved embryos, resulting in one singleton pregnancy. The remaining couple underwent a second oocyte donation cycle without success. All female recipients tested HIV-EIA negative post-ET. All babies (n = 2) tested HIV-DNA negative at birth and 3 months postpartum (the triplet pregnancy is ongoing). Conclusion: Oocyte donation should be considered as an alternative for childbearing for HIV-serodiscordant couples where conventional IVF-ICSI has failed due to difficulties using conventional IVF in female partners." @default.
