- W2000419319 abstract "To the Editor: I report a case of pseudoaneurysm of the transverse cervical artery, a branch of the thyrocervical trunk [1], as a complication of percutaneous cannulation of the internal jugular vein. This was attempted on the right side using an Arrow-Howes Registered Trademark 7Fr triple-lumen central venous catheterization set with Arrow Raulerson Registered Trademark spring wire introduction syringe (CS 14703E; Arrow International Inc., Reading, PA) in a patient presenting for coronary artery revascularization. Arterial puncture was not identified until after placement of the catheter. After digital compression, there was no evidence of hematoma formation. However, on completion of surgery a large hematoma was apparent but resolved over the next 3 days. A 4-cm diameter pulsatile supraclavicular mass with a bruit on auscultation remained. By digital subtraction angiography, pseudoaneurysm arising from the transverse cervical artery was diagnosed Figure 1. A decision was made that the radiologists would attempt to embolize the offending artery, rather than subjecting the patient to exploratory surgery. However, as the pulsation had stopped by the sixth postoperative day, no further intervention was undertaken, and the patient made an uneventful recovery.Figure 1: Jet of blood (arrowed) from transverse cervical artery into pseudoaneurysm.Apart from the obvious hazard of carotid artery puncture, there have been a number of reports of damage to other arteries in the neck [2-6]. The introduction of the Raulerson syringe Registered Trademark, while improving patient safety and operator convenience [7], may reduce the recognition of arterial puncture. Whether subsequent systemic heparinization increases the risk of hematoma development remains controversial; one uncontrolled study suggested it did not [8]. In conclusion, in this case the condition was self-limiting, perhaps due to the caliber of the artery. R. G.
