- W2000579743 abstract "Objective: The aim of the present study was to assess the role of adaptive structural and functional changes in the increase of vascular resistance in essential hypertension patients (WHO I-II). Design and Method: Two series of the investigations were performed on 20 healthy subjects and 45 essential hypertension patients (WHO I-II). In the first series of the investigation, the minimal haemodynamic resistance (Rmin.) was calculated on the basis of the measurements of systemic arterial pressure (SAP) and maximal blood flow (Imax) in the forearm skeletal muscles during reactive hyperemia after arterial occlussion (AO) lasting for 7 min. In the second series, SAP, Imax and Rmin. were determined after AO of 7 min. following the venous congestion in the forearm arterial vessels. Venous congestion was performed by stepwise increasing venous pressure in the forearm veins till venous volume reached maximum. Venous volume changes and blood flow were determined in the upper third of forearm by venous occlusion plethysmographic method. Results: It was stated that the Rmin. after AO of 7 min. is significantly greater in the forearm of essential hypertension patients (3,2 ± 0,05 units) than in healthy subjects (2,7 ± 0,2 units). Venous congestion before AO of 7 min. evokes a decrease in Rmin. both in healthy subjects and in hypertension patients. One must note that the Rmin. of hypertension patients (2,15 ± 0,05 units) becomes equal with the Rmin. of healthy subjects (2,25 ± 0,1 units). So, the tonic activity retained in arterial smooth muscles after AO of 7 min. is greater in hypertension patients than in healthy subjects. Conclusions: The increased myogenic activity of vascular smooth muscles is a decisive factor increasing Rmin. in essential hypertension patients (WHO I-II). Venous congestion before AO ensuring the retrograde increase of transmural pressure and passive distending of forearm arteries during AO, allows to determine the functional and the structural components of minimal regional haemodynamic resistance." @default.
