- W2000600600 abstract "背景・目的：日本救急医学会は延命治療の中止基準を本邦の医学会として初めて策定した。本研究では，同学会の「終末期医療に関する提言（ガイドライン）」策定直前の救急医療現場において，延命治療の中止に関わる問題点を，人工呼吸器に焦点を当て，救急医の経験と認識に基づいて探索的に調査することを目的とした。対象・方法：対象は救急医35名（男性31名，女性4名；年齢中央値49歳）。データは2006～2007年に個別の半構造化インタビューによって収集し，データ収集と分析にはgrounded theory approachの手法を用いた。結果：末期患者において人工呼吸器の中止を通常の臨床上の選択肢としていた医師はおらず，その理由として，1）警察の介入・報道問題，2）家族関連問題，3）医師側の心理的障壁，4）医学的要因，という直接要因群の存在が示された。一方，他の治療法は中止も選択肢であることが示された。考察：延命治療の中止基準が不在であったなか，近年の具体事例への警察の介入とそれに関する報道内容が実質的な「社会の許容限度」を臨床医に知らせる形になっていたことが示された。しかし，警察と報道の対応に問題があったため，その「許容限度」という解釈にも問題があることが示唆された。学会「ガイドライン」によってこの問題が緩和される可能性は示唆されたが，人工呼吸器の中止という行為を「縮命への作為」と認識することを中核とする「医師の心理的障壁」と，「医学的要因」の構成要素である「中止によって患者に苦痛を与える懸念」という問題は残される可能性が示唆された。" @default.
